Our model of support is aimed at outcome-based support, this means that we support people who live with a range of disabilities to achieve their wishes, goals, and ambitions this could be something small or big.
How our model works
Outcome-based support is indeed a person-centered approach that focuses on the individual’s personal goals. It’s about understanding and respecting each person’s unique needs, aspirations, and circumstances. Here’s how it relates to our support:
In essence, outcome-based support is about empowering individuals to live their lives to the fullest, in a way that aligns with their personal goals and aspirations. It’s about providing the right support at the right time, to help individuals achieve their desired outcomes. It’s about respecting individuality, promoting autonomy, and fostering positive, informed decision-making. It’s about making a real, meaningful difference in people’s lives.
We have established four core values to guarantee fair treatment for all:

Why is our model important?
Our model is crucial for the individuals we assist, as it empowers them to develop their own skills and abilities, thereby enhancing their everyday life skills within a person-centered framework.
It is also vital in ensuring that the people we support receive a positive service, fostering a trusting relationship where their views and wishes are acknowledged, giving them the motivation to pursue new goals.
Regarding active support, our philosophy has always been to assist rather than do for, ensuring that our team comprehensively understands active support. This approach is consistently applied across our services, encouraging individuals to undertake tasks independently or with minimal assistance, thus boosting their self-esteem and aiding in the acquisition of daily living skills.
How will we implement this?
Our model is integrated each time we assist someone, whether in a hub setting or in the community as one-to-one support, tailored to the individual's abilities and needs.
Each person we assist is assigned a key worker who helps them achieve their targeted outcomes. Upon reaching an outcome, a reflection session is conducted with the individual and their family members.
We reflect on three aspects: what was successful, what was learned, and what could be altered to either enhance their abilities or sustain their achievements. These reflections can be revisited whenever necessary. We also conduct reviews with the client and family members to establish new objectives they aspire to accomplish.
